Ítem Adipocitos, obesidad visceral, inflamación y enfermedad cardiovascular(Sociedad Colombianade Cardiología, 2010-10-01) Manzur, FernandoLa obesidad es un importante problema de salud a nivel mundial. Se considera el resultado de la combinación de factores genéticos, alimentación inadecuada y falta de actividad física regular. La ingestión de una dieta de alta densidad energética, es la principal causa de obesidad visceral o central, ya que el exceso de energía se almacena en los adipocitos, que aumentan en tamaño y en número, o ambos, en especial los viscerales, produciendo un incremento en la tasa de lipólisis, que a su vez, estimula la secreción de citoquinas por leucocitos, macrófagos y adipocitos, y conduce a estado proinflamatorio, resistencia a la insulina y disfunción endotelial. Esta última, favorecida por el proceso inflamatorio, puede ser el vínculo de unión entre la obesidad y la enfermedad cardiovascular. The etiologic agent was identified in 87.4% of cases and predominantly consisted of drugs (31.2%), foods (23.3%), and insect stings (14.9%). The main symptom categories observed during the acute episodes were cutaneous (94.0%) and respiratory (79.0%). The majority of patients (71.6%) were treated initially by a physician (office/emergency room) within the first hour after the reaction occurred (60.2%), and 43.5% recovered in the first hour after treatment. Most patients were treated in an emergency setting, but only 37.3% received parenteral epinephrine alone or associated with other medication. However, 80.5% and 70.2% were treated with corticosteroids or antihistamines (alone or in association), respectively. A total of 12.9% of the patients underwent reanimation maneuvers, and 15.2% were hospitalized. Only 5.8% of the patients returned to the emergency room after discharge, with 21.7% returning in the first 6 hours after initial treatment.Ítem Desenlaces clínicos en una cohorte de pacientes con síndrome coronario agudo y administración intracoronaria de tirofiban(Elsevier, 2015-01-01) Carrillo Gómez, Diana CristinaIntroduction: Glycoprotein IIb/IIIa receptor inhibitors reduce major adverse cardiovascularevents (MACE) in patients with acute coronary syndrome.Objective: To determine the major adverse cardiovascular events and safety of intracoronaryand intravenous tirofiban in patients with acute coronary syndrome with percutaneous coronaryintervention (PCI) compared to a group of patients without this medication.Material and methods:Prospective cohort. Adults with acute coronary syndrome from January2010 to December 2012 were included. A proportional hazard regression model after 1 monthof follow up where the risk of MACE and the interest variable was intracoronary and intravenoustirofiban were assessed.Results: 382 patients were included. 46% (174 patients) received intracoronary tirofiban. Theaverage age was 65 years vs. 60 years in the group of non-tirofiban users (p = 0.00). The rate ofMACE at the first month was od 13/1000 events and 15/1000 events respectively in the grouptirofiban and non-tirofiban (log rank test 0,935). The tirofiban use was neither related to a lowerincidence of MACE (HR 1.09 95% IC 0.72 1.65) nor to major bleeding after the first month offollow up (1.72% vs. 2.88% respectively, p = 0.456).Conclusions: Intracoronary and intravenous tirofiban in patients with acute coronary syndromewas neither related to lower incidence of MACE nor to bleeding events; however, it should betaken into account that other clinical variables and the severity of unquantified coronary eventsmight influence the results. © 2015 Sociedad Colombiana de Cardiología y Cirugía Cardiovascular.Ítem Echocardiographic assessment of valve prostheses in the pediatric population(Sociedad Colombiana de Cardiologia y Cirugia Cardiovascular, 2014-01-01) Santos, Raúl D.En niños con insuficiencia o estenosis valvular significativa, la intervención de la válvula, ya sea valvuloplastia o reemplazo valvular, suele ser inevitable. El seguimiento de estos pacientes puede ser complejo; los síntomas iniciales de disfunción valvular protésica son inespecíficos y en ocasiones es difícil diferenciar entre los efectos de la disfunción valvular, las patologías no cardíacas, la disfunción ventricular y la hipertensión pulmonar. Aunque el examen físico puede alertar al clínico acerca de alteraciones, se requieren otros métodos diagnósticos para evaluar la función de la prótesis. According to the World Health Organisation, the prevalence of fasting hyperglycaemia in the region of the Americas in 2014 was 9.3% in men and 8.1% in women. The countries with the highest prevalence of diabetes mellitus in adults ≥ 18 years were: Guyana, Surinam, Chile, and Argentina. The article defines the architecture and functions of the global resource scheduler, as well as the quality index for scheduling, the signal to interference-plus-noise ratio (SINR). The proposed approach divides the scheduling task into two components: a distributed one, with local, short-term scope; and a centralized one, with global, medium-term scope. The optimization model considers a set of slack variables for guaranteeing feasibility. This allows the service provider to fully satisfy the users' service demands. The model type is mixed, non-linear, which demands large computational power for an exact solution. So an approximate strategy is used, in order to decouple the search space.Ítem Post-chikungunya chronic inflammatory rheumatism: Results from a retrospective follow-up study of 283 adult and child cases in La Virginia, Risaralda, Colombia(F1000 Research Ltd., 2016-12-25) Ramírez Jaramillo, ValeriaObjective: There are limited studies in Latin America regarding the chronic consequences of the Chikungunya virus (CHIK), such as post-CHIK chronic inflammatory rheumatism (pCHIK-CIR). We assessed the largest cohort so far of pCHIK-CIR in Latin America, at the municipality of La Virginia, Risaralda, a new endemic area of CHIK in Colombia. Methods: We conducted a cohort retrospective study in Colombia of 283 patients diagnosed with CHIK that persisted with pCHIK-CIR after a minimum of 6 weeks and up to a maximum of 26.1 weeks. pCHIK cases were identified according to validated criteria via telephone. Results: Of the total CHIK-infected subjects, 152 (53.7%) reported persistent rheumatological symptoms (pCHIK-CIR). All of these patients reported joint pains (chronic polyarthralgia, pCHIK-CPA), 49.5% morning stiffness, 40.6% joint edema, and 16.6% joint redness. Of all patients, 19.4% required and attended for care prior to the current study assessment (1.4% consulting rheumatologists).Ítem Prevalencia de trastornos mentales en una población que demanda servicios de salud en una IPS de baja complejidad y su asociación con la ideación suicida y la discapacidad percibida(Universidad de Antioquia, Facultad Nacional de Salud Pública, 2012-06-20) Rincón Hoyos, Hernán GilbertoConocer los trastornos mentales más frecuentes y el impacto en la percepción de discapacidad e ideación suicida en una Institución prestadora de servicios de salud de baja complejidad en Cali. Metodología: se realizó un estudio transversal con 254 pacientes a los que se les aplicó el instrumento prime-md. En el análisis se utilizó estadística descriptiva para determinar los trastornos más frecuentes y un análisis múltiple por regresiones logística y de Poisson con varianza robusta para determinar la influencia de los trastornos mentales en la discapacidad e ideación suicida. Resultados: la mayoría de los pacientes eran del sexo femenino, adultos jóvenes y maduros. La depresión estuvo presente en 66.8% de los casos, seguido del trastorno de somatización y la ansiedad. Casi la mitad de los pacientes habían tenido ideas suicidas alguna vez en la vida y 3 de cada 4 refirieron algún tipo de discapacidad. Después de ajustar por covariables, los trastornos ansiosos-depresivos mostraron una alta asociación con la ideación suicida y la discapacidad percibida, que es sobrestimada por la regresión logística. Conclusión: la depresión y la ansiedad son los trastornos mas frecuentes y tienen una alta asociación con la ideación suicida y la discapacidad y es por esto necesario realizar tamizaciones en la atención del adulto en servicios de baja complejidad. ESBL confirmatory test, antibiotic susceptibility, and molecular epidemiology were performed in these E.Ítem Safety of Parecoxib in Asthmatic Patients with Aspirin-Exacerbated Respiratory Disease(2011-09-01) Serrano Reyes, Carlos DanielBACKGROUND Aspirin-exacerbated respiratory disease (AERD) affects a subset of patients with asthma. Cyclooxygenase 2 inhibitors are a safe alternative in patients with AERD. Parecoxib is the first cyclooxygenase 2 selective drug for parenteral administration, especially useful after surgery thanks to its analgesic power. The aim of the study is to assess the tolerance of parecoxib (Dynastat; Pfizer) given by intramuscular route in patients with AERD. METHODS Patients evaluated were referred to the Pneumology and Respiratory Allergy Department of the Hospital Clinic (Barcelona, Spain) for asthma exacerbations precipitated by 2 or more different non-steroidal anti-inflammatory drugs (NSAIDs). AERD was confirmed by a nasal challenge test with aspirin. Patients were challenged with parecoxib, and urine samples were collected to measure the leukotriene E(4) concentration. RESULTS Ten patients were challenged with parecoxib. No symptoms were reported with any of the administered doses, and there were no signs of immediate or delayed hypersensitivity. There were no alterations in the forced expiratory volume in 1 s or in acoustic rhinometry measurements. No significant differences between leukotriene E(4) levels were detected. CONCLUSION The drug was well tolerated by all patients, with no adverse reactions. In Latin America and Colombia where tuberculosis is endemic, it represents between 5 and 30% of brain tumours. A 53-year-old Colombian woman was admitted to a third-level hospital in Cali, Colombia, after reporting loss of consciousness, headache, paresthesia, and flight of ideas for a two-week period. Imaging studies showed a left frontal mass of malignant appearance whose first possible diagnosis was metastatic neoplasiaorglioma. With the initial results, absence of history of chronic infectious diseases and a history of thyroidectomy, a surgical procedure was carried out and a histopathological and molecular evaluation was conducted. The pathology report noted necrotizing granulomatous inflammation and tissue staining and molecular tests for detection of M. tuberculosis were positive and the patient was managed with anti-tubercular treatment. Intracranial masses are frequently targeted as a malignant neoplastic disease for surgical treatment. Considering an infectious etiology must be a diagnostic option. © 2013 Association of Basic Medical Sciences of FB&H.
