    Design and Development of the Institutional Registry of Pulmonary Hypertension in Fundacion Valle del Lili, a University Hospital of Reference in Latin America

    In the present, the epidemiologic characteristics of Pulmonary Hypertension (PH) are not completely defined, since regional differences exist conditioned by genetic, geographical, environmental and socioeconomic factors that determine the clinical behavior and prognosis of patients with PH. Objective: To elaborate an institutional registry of HP in Fundación Valle de Lili (FVL) within the framework of PH Clinic including cases belonging to group 1 and 4 characterizing the presentation, impact and the natural history of disease, serving this as a support for the development of a regional registry in the future.

    Epidermal Growth Factor Receptor Mutation In Patients With Non-Small Cell Lung Carcinoma In Fundacion Valle Del Lili, A University Hospital Of Reference In Latin America

    Epidermal Growth Factor Receptor (EGFR) exon 19-deletion and exon 21 L858R mutation is found in 10% of Non-Small Cell Lung Carcinoma (NSCLC) in Caucasians and in 20 to 40% in Asians. The use of Tyrosine Kinase Inhibitors (TKI) in these genetic alterations has demonstrated a greater response and progression free survival in 9-15 months. Objective: To describe the experience in EGFR identification in Fundación Valle del Lili (FVL). Materials and Methods Between June/2013 and Sep/2015, 95 samples were analyzed. A microdissection of tumor areas was performed as well as DNA extraction with QIAamp FFPE Kit(Qiagen®) and amplification of exons 18, 19, 20 and 21 by PCR. It was visualized through microfluidic electrophoresis with Agilent Bioanalyzer® System. Results 98 samples were analyzed. Amplified DNA was obtained in 97% (95) of the cases. The mean age was 65,4±12 years, 63,2% (60) females and 36,8% (35) males. The mutation was observed in 25% (24), exon 19 in 10,5% and exon 21 in 14,7%. 83,1% of the tumors were adenocarcinomas, 70,5% in stage IV and 29% of the cases with a mutation was receiving TKI. Mortality was 14,7% .

    Fatal Hemoptysis Secondary To Fibro-Inflammatory Igg4-Related Lung Disease: A Case Report

    IgG4-related disease (IgGe-RD) is a fibro-inflammatory condition that affects nearly every organ; it was initially describe in the pancreas, by lesions with plasma cell infiltration with IgG4+. It presents a dense lymphoplasmocytic infiltrate, occasionally pseudotumoral, with fibrotic stroma with a storiform pattern and obliterative phlebitis, the lung representation was characterized in 2011, two thirds of the patients had increased IgG4 levels, there is a response to steroids in early stages of the disease.

    In the present, the flexible bronchoscopy (FB) has become the procedure of election to explore the airways fue to its diagnostic and therapeutic capacity in pulmonary pathology. In the critically ill patient this tool gains advantage since it provides the availability of being used at the bedside with few complications. In this study we describe the indications, results and complications associated to the performance of FB in the Intensive Care Unit of Fundación Valle del Lili, a University Hospital Reference in Cali, Colombia during 2013.

    The Central Airway Obstruction (CAO) has malignante and non-malignat causes; the most frequent is pulmonary cancer. Usually present cough and progressive dyspnea, even respiratory failure. Therapy must ensure and restore the airway. The chosen technique depends on the cause, type and severity of the lesion, as well as the technological availability and operator’s ability. Our objective is to describe the experience in Therapeutic Bronchoscopy in Fundación Valle del Lili.

    A 40-Year-Old Patient With Autoimmune Hepatitis And Portopulmonary Hypertension With Miliary Tuberculosis. Challenges Of Drug Treatment: A Case Report

    Autoimmune hepatitis (AIH) is a rare disease with prevalence of 1.07-1.9 per 100.000 in Europe affects preferentially females and is characterized by idiopathic hepatitis, increased seric gamma globulins, and circulating non-organ-specific antibodies and the comerstone of the therapy is immunosuppression. The complications of AIH are the same as in any other chronic liver disease including hepatopulmonary syndrome (HPS) and portopulmonary hypertension (PPH). We discuss a case report of PPH and milliary tuberculosis (tb) and interactions between sildenafil and antituberculosis treatment.
