    Trauma Severity and Not the Socio-Economic Variables Determine Survival after Penetrating Trauma in a Medium-Income Country

    Survival after severe trauma is lower in low- and middle-income countries. Quality of pre-hospital care, lack of insurance, and differential access to trauma care are possible explanations. The aim of this study was to evaluate the effect of lack of insurance, pre-hospital times, and the nature of the hospital (public or private) in survival after violent penetrating trauma in a middle-income country.

    Postprandial lipemia induces endothelial dysfunction and higher insulin resistance in healthy subjects

    Objective: To assess the effect of postprandial lipemia on endothelial function, insulin resistance, and lipid profile in healthy subjects. Patients and methods: A prospective', interventional study in 14 healthy young men aged 18-25 years who were given a high-fat meal. Endothelial function was measured using flow-mediated dilation (FMD) in the brachial artery, flow velocity, mean arterial pressure and serum nitrite/nitrate levels (NO 2/NO 3). Glucose, insulin, total cholesterol, and triglyceride levels were also tested.

However, data about the variability and reproducibility of this technique should be collected before it can be used. Objective: To assess reproducibility of RRS for detection of total β-carotene levels in the skin of Colombian adults.

    Monitoring the microcirculation in the critically ill patient: current methods and future approaches

    To discuss the techniques currently available to evaluate the microcirculation in critically ill patients. In addition, the most clinically relevant microcirculatory alterations will be discussed. Review of the literature on methods used to evaluate the microcirculation in humans and on microcirculatory alterations in critically ill patients. In experimental conditions, shock states have been shown to be associated with a decrease in perfused capillary density and an increase in the heterogeneity of microcirculatory perfusion, with non-perfused capillaries in close vicinity to perfused capillaries. Techniques used to evaluate the microcirculation in humans should take into account the heterogeneity of microvascular perfusion. Microvideoscopic techniques, such as orthogonal polarization spectral (OPS) and sidestream dark field (SDF) imaging, directly evaluate microvascular networks covered by a thin epithelium, such as the sublingual microcirculation. Laser Doppler and tissue O(2) measurements satisfactorily detect global decreases in tissue perfusion but not heterogeneity of microvascular perfusion. These techniques, and in particular laser Doppler and near-infrared spectroscopy, may help to evaluate the dynamic response of the microcirculation to a stress test. In patients with severe sepsis and septic shock, the microcirculation is characterized by a decrease in capillary density and in the proportion of perfused capillaries, together with a blunted response to a vascular occlusion test.

    Índice de shock como factor predictor de mortalidad en el paciente con trauma penetrante de tórax

    El trauma es considerado un problema de salud pública. Es una de las causas de mayor discapacidad y mortalidad en Latinoamérica y Colombia no es la excepción. El objetivo de este estudio fue evaluar si el índice de shock, resultado de la frecuencia cardiaca sobre la tensión arterial sistólica, es útil para predecir la mortalidad a 24 horas, en pacientes con trauma penetrante de tórax atendidos en el servicio de urgencias de un hospital universitario de Colombia.

    Hyperferritinemic syndrome: Still’s disease and catastrophic antiphospholipid syndrome triggered by fulminant Chikungunya infection: a case report of two patients

    There are four medical conditions characterized by high levels of ferritin, the macrophage activation syndrome (MAS), adult onset Still’ s disease (AOSD), catastrophic antiphospholipid syndrome (CAPS), and septic shock, that share similar clinical and laboratory features, suggesting a common pathogenic mechanism. This common syndrome entity is termed “the hyperferritinemic syndrome.” Here, we describe two different cases of hyperferritinemic syndrome triggered by Chikungunya fever virus infection: a 21-year-old female with SLE and a 32-year-old male patient who developed AOSD after the coinfection of dengue and Chikungunya viruses.

    Echocardiographic assessment of valve prostheses in the pediatric population

    En niños con insuficiencia o estenosis valvular significativa, la intervención de la válvula, ya sea valvuloplastia o reemplazo valvular, suele ser inevitable. El seguimiento de estos pacientes puede ser complejo; los síntomas iniciales de disfunción valvular protésica son inespecíficos y en ocasiones es difícil diferenciar entre los efectos de la disfunción valvular, las patologías no cardíacas, la disfunción ventricular y la hipertensión pulmonar. Aunque el examen físico puede alertar al clínico acerca de alteraciones, se requieren otros métodos diagnósticos para evaluar la función de la prótesis. La ecocardiografía Doppler es el método de elección no invasivo para estudiar la función valvular y pese a que muchas de las mediciones y parámetros de normalidad y anormalidad son extrapolados de estudios en adultos, se han tratado de definir estándares en pacientes pediátricos. Este documento ofrece una revisión acerca de las técnicas usadas en el estudio de las válvulas protésicas con base en literatura científica, consensos internacionales y opiniones de expertos.

    Epidemiology of diabetes mellitus in South America: The experience of Colombia

    According to the International Diabetes Federation, 8.3% of the world population suffers from diabetes mellitus, and it is expected that the number of individuals with the disease will increase to over 592 million. In South and Central America, it is estimated that the increase in the number of cases diagnosed in the period from 2013 to 2035 will be 59.8% (from 24 to 38.5 millions). According to the World Health Organisation, the prevalence of fasting hyperglycaemia in the region of the Americas in 2014 was 9.3% in men and 8.1% in women. The countries with the highest prevalence of diabetes mellitus in adults ≥ 18 years were: Guyana, Surinam, Chile, and Argentina. In Colombia, the prevalence of type 2 diabetes mellitus is variable, depending on the population range assessed and the diagnostic criteria used. © 2016 Sociedad Española de Arteriosclerosis

    Desenlaces clínicos en una cohorte de pacientes con síndrome coronario agudo y administración intracoronaria de tirofiban

    Introduction: Glycoprotein IIb/IIIa receptor inhibitors reduce major adverse cardiovascularevents (MACE) in patients with acute coronary syndrome.Objective: To determine the major adverse cardiovascular events and safety of intracoronaryand intravenous tirofiban in patients with acute coronary syndrome with percutaneous coronaryintervention (PCI) compared to a group of patients without this medication.Material and methods:Prospective cohort. Adults with acute coronary syndrome from January2010 to December 2012 were included. A proportional hazard regression model after 1 monthof follow up where the risk of MACE and the interest variable was intracoronary and intravenoustirofiban were assessed.Results: 382 patients were included. 46% (174 patients) received intracoronary tirofiban. Theaverage age was 65 years vs. 60 years in the group of non-tirofiban users (p = 0.00). The rate ofMACE at the first month was od 13/1000 events and 15/1000 events respectively in the grouptirofiban and non-tirofiban (log rank test 0,935). The tirofiban use was neither related to a lowerincidence of MACE (HR 1.09 95% IC 0.72 1.65) nor to major bleeding after the first month offollow up (1.72% vs. 2.88% respectively, p = 0.456).Conclusions: Intracoronary and intravenous tirofiban in patients with acute coronary syndromewas neither related to lower incidence of MACE nor to bleeding events; however, it should betaken into account that other clinical variables and the severity of unquantified coronary eventsmight influence the results. © 2015 Sociedad Colombiana de Cardiología y Cirugía Cardiovascular.

    Mid-Term frequency domain scheduler for resource allocation in wireless mobile communications systems.

    This article approaches the dynamic resource allocation problem for the downlink of a wireless mobile communication system (WMCS). The article defines the architecture and functions of the global resource scheduler, as well as the quality index for scheduling, the signal to interference-plus-noise ratio (SINR). The proposed approach divides the scheduling task into two components: a distributed one, with local, short-term scope; and a centralized one, with global, medium-term scope. The optimization model considers a set of slack variables for guaranteeing feasibility. This allows the service provider to fully satisfy the users' service demands. The model type is mixed, non-linear, which demands large computational power for an exact solution. So an approximate strategy is used, in order to decouple the search space.

    Objective: to analyze avoidable mortality between 1998 and 2007 in the border states of Northern Mexico to evaluate, indirectly, the quality of the region's health care services. Methodology: the information on mortality provided by the National Health Information System (sinais) was analyzed. The unit of analysis was the basic cause of death. The cause was coded in accordance with the ICD-10.Avoidable mortality was classified according to Gomez’s death causes catalog. Likewise, an exploratory analysis was conducted which focused on the relationship between avoidable mortality and eligibility and the socioeconomic level of the municipalities in which the deaths took place.

    Prevalencia y factores de riesgo para infecciones del tracto urinario de inicio en la comunidad causadas por Escherichia coli productor de betalactamasas de espectro extendido en Colombia

This study aims to determine the prevalence and risk factors associated with community-onset UTI (CO-UTI) caused by extended-spectrum β-lactamase (ESBL)-Producing Escherichia coli in Colombia. Materials and methods A case-control study was conducted between August and December of 2011 in three Colombian tertiary-care institutions. All patients who were admitted to the Emergency Department with a probable diagnosis of CO-UTI were invited to participate. All participating patients were asked for a urine sample.     Post-chikungunya chronic inflammatory rheumatism: Results from a retrospective follow-up study of 283 adult and child cases in La Virginia, Risaralda, Colombia

    Objective: There are limited studies in Latin America regarding the chronic consequences of the Chikungunya virus (CHIK), such as post-CHIK chronic inflammatory rheumatism (pCHIK-CIR). We assessed the largest cohort so far of pCHIK-CIR in Latin America, at the municipality of La Virginia, Risaralda, a new endemic area of CHIK in Colombia. Methods: We conducted a cohort retrospective study in Colombia of 283 patients diagnosed with CHIK that persisted with pCHIK-CIR after a minimum of 6 weeks and up to a maximum of 26.1 weeks. pCHIK cases were identified according to validated criteria via telephone. Results: Of the total CHIK-infected subjects, 152 (53.7%) reported persistent rheumatological symptoms (pCHIK-CIR). All of these patients reported joint pains (chronic polyarthralgia, pCHIK-CPA), 49.5% morning stiffness, 40.6% joint edema, and 16.6% joint redness. Of all patients, 19.4% required and attended for care prior to the current study assessment (1.4% consulting rheumatologists).

    Valoración de la inflamación en el asma

    En 1960 se descubrió que el asma es una enfermedad inflatoria; este descubrimiento llevó a una revolución terapéutica, además de la bronco constricción, se comenzó a tratar la inflamación. El proceso inflamatorio en el asma es complejo, y algunos de sus mecanismos no están totalmente entendidos, es claro su papel central en la fisiopatología y la progresión enfermedad. Por todo ello, sería conveniente poder valorar la inflamación en el seguimiento de los pacientes. La presencia de inflamación en el asma se comprobó por estudios histopatológicos realizados en pacientes con asma letal. En sus vías aéreas se evidenciaba infiltración de neutrófilos, eosinófilos y mastocitos; engrosamiento de la membrana basal: pérdida de la integridad del epitelio, y obstrucción del lumen bronquial por moco.
